Responsible for all aspects of the patient intake process including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITY
1. Performs all daily patient referral and intake operations.
2. Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
3. Performs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Ensures seamless transition of patients to home care by providing direct oversight of
patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
6. Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
7. Builds and monitors community and customer perceptions of Freedom Home Health and Hospice Care Services Inc. as a high quality provider of services.
8. Gathers, collates, and reports referral statistics including key customer referral trends.
9. Maintain comprehensive working knowledge of Freedom Home Health and Hospice Care Services Inc., contractual relationships and ensure that patients are admitted according to contract provisions.
10. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Freedom Home Health Care.

Clinical Duties:
1.Measuring and recording patient information like blood pressure, pulse, temperature, and weight.
2. Preparing patients, setting up equipment, and assisting physicians during exams and procedures.
3. Drawing blood, administering injections, performing EKG’s and collecting specimens.
4. Ensuring a clean and safe environment for patients and staff
5. Under the direction of a healthcare professional, may administer medications or vaccinations.
POSITION QUALIFICATIONS
1. High school graduate or equivalent.
2. Certified Medical Assistant (CMA)
3. Two (2) years data entry experience. Previous computer experience, preferably in home health or similar operation.
4. Previous health care related experience.
SKILLS REQUIRED
1. Ability to type 50 words per minute
2. Word processing skills
3. Personal computer skills
4. Demonstrates good communication, negotiation, and public relations skills
